Romance here is on the lighter side at the beginning, and that IS due to some circumstances which make it hard for the MC to feel emotions (minor spoiler, but it was legitimately mentioned early).
But later, the romance is blossoming. I personally suggest this.

Pretty sure he has no sister, only a mother as her family. I think (big think) that the love interests do know each other, or something similar. So it's not 1 main love interest + countless side h*es. But I don't remember a lot, fever dream you might say👍
